About the role
This role leads the development and delivery of ILP’s neighbourhood-based health and wellbeing offer, supporting communities to improve their health through connection, activity, and community-led solutions.
You will lead My Health, My Way as a core programme, delivering it across wellbeing hubs and expanding it into neighbourhood and community settings to increase access and engagement.
You’ll work closely with local residents, community groups, and partners to create opportunities that support physical activity, social connection, and prevention—particularly for older people and those living with long-term conditions.
This is a hands-on role combining community development, programme delivery, and leadership to deliver high-quality, inclusive services.

Key Responsibilities
🌍 Neighbourhood health and community engagement
- Build strong relationships with local residents, community groups, and partners
- Support community-led health and wellbeing activities using local strengths and assets
- Encourage physical activity, social connection, and peer support, particularly for older people
- Ensure community insight and lived experience shape delivery and development
🔗 Integration and service development
- Support joined-up working between neighbourhood delivery and wellbeing hubs
- Contribute to service improvement, learning, and evaluation
- Champion ILP values and collaborative ways of working
💚 Programme delivery (My Health, My Way)
- Lead delivery of My Health, My Way across hubs and neighbourhood settings
- Expand delivery into community venues to improve access and engagement
- Plan and deliver safe, inclusive, high-quality sessions (group and one-to-one)
- Work with partners to connect the programme into wider health services
🤝 Partnership and system working
- Build relationships with VCSE, statutory services, and community leaders
- Represent ILP in local forums and partnerships
- Develop joint initiatives to improve health and reduce inequalities
👥 Leadership and delivery management
- Manage and support staff, volunteers, and students
- Coordinate delivery across neighbourhood locations and sites
- Monitor activity and outcomes using systems and reporting tools
- Support funding, sustainability, and programme development
🏡 Community spaces and place-based work
- Support the development of community spaces for health and wellbeing
- Increase access to local opportunities for prevention, activity, and connection
- Enable communities to take ownership of local initiatives
